What We do
Our services offer support to adults living with a range of diagnosis within the community which includes but not limited to Elderly Care, physical Disability, individuals living with Dementia and Young adults 18+. We also support individuals stepping down from secure settings and those that may have experienced previous placement breakdowns.
OUR SERVICES
Community Outreach
We support individuals to engage in meaningful activities of their choice and help them to achieve positive outcomes. These include but not limited to support to go on walking groups, horse riding, travel training, domestic chores and sit in service.
Live-In Services
We provide carers who can live with the client 24/7 and work with the client assisting them to meet personal needs. The care is planned by experienced assessors on a multi-disciplinary approach – while incorporating the wishes, views and feelings of the individual and their families.
Respite Care
We offer carers a break from their role as carers for either a day, weekend or for as long as they need. This can be facilitated within the family home.
Domiciliary Care
We offer person centred support for individuals living with:
- Physical disabilities
- Higher Support needs
- Dementia
- Elderly support
- Young adults
- End of life
Training and Compliance
